By using the Lilly Insulin Lispro injection Vial Self-Pay Savings Card Program ("Program") and using the Lilly Insulin Lispro Vial Self-Pay Savings Card ("Card"), you attest that you meet the eligibility criteria, and you agree to comply with the terms and conditions described below:

#### Card Eligibility:
(1.) You have been prescribed Insulin Lispro injection vial (10mL) consistent with FDA approved product labeling;
(2.) You are not enrolled in any state, federal, or government funded healthcare program, including, without limitation, Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, TRICARE®/CHAMPUS, or any state prescription drug assistance program;
(3.) You are a resident of the United States or a United States Territory (Puerto Rico, Guam, U.S. Virgin Islands, Northern Mariana Islands, American Samoa); and  
(4.) You are 18 years of age or older.

#### Card Terms and Conditions
You must have a prescription for an approved use consistent with FDA-approved product labeling to pay as little as $35 for a 1-month prescription fill of your Insulin Lispro injection vial (10mL). One month is defined as 30-days. Card savings are subject to a maximum monthly savings based on wholesale acquisition cost minus self-pay price of $35 per prescription fill of each Insulin Lispro injection vial (10mL).

#### Additional Program Terms and Conditions
You understand and agree this Program and Card is for self-paying (cash) patients and operates outside of any health insurance program. You agree not to seek payment or accept reimbursement for any out-of-pocket costs for Insulin Lispro injection vials purchased with this Card.
